WHAT ARE YOUR OFFICE HOURS?

Our office hours are 8:30 AM – 5:00 PM CST, Monday through Thursday, and 8:30 to 12:00 PM on Friday.


HOW DO I REQUEST AN APPOINTMENT?

Fill out our online Appointment Request Form to request an appointment -- OR -- Call 979-299-3100 to speak with our office staff.


WHO DO I CALL FOR BILLING RELATED QUESTIONS?

Please contact our billing department at 979-299-3100 OR fill out our Online Contact Form and we will respond to you as soon as possible.


WHAT TYPES OF INSURANCE DO YOU ACCEPT?

We accept most types of insurance, including:

If you do not see your insurance carrier here, we will most likely accept your coverage. You can verify benefits by calling the number on the back of your card, or by using your insurance carrier's website for in network or out of network benefits.

If you are having difficulty doing this or are unsure of you coverage, please call our office at 979-849-9557 and we will be more than happy to assist you.

WHAT IF I NEED A PRESCRIPTION?

New prescription requests may be made during your appointment with Dr. Eggleston or by contacting our office. One of our nursing staff will forward your request to Dr. Eggleston for approval.

If you are needing a prescription refilled, please contact your pharmacy and they will fax our office your request. Prescription requests received from your pharmacy after 12:00 noon on Friday will not be refilled until Monday.

There will be no after hours or weekend refills.


WHAT IF I NEED COPIES OF MEDICAL RECORDS OR DISABILITY PAPERS FILLED OUT?

If you are needing copies of your medical records, please contact our office and please allow 48-72 hours for our staff to make those copies for you.

Disability paperwork is addressed on Friday mornings. Please bring your paperwork into office NO LATER THAN WEDNESDAY for them to be complete on that following Friday morning.

REMINDER: Office closes by NOON on FRIDAY. There is a $10 fee for completion of disability paperwork payable at the time of the request.
